Information About therapy
The Intake Process
Complete the linked form below to contact me. This is a secure, encrypted form and will include your contact information, insurance information (to verify benefits), and asks the main reason you’re seeking therapy at this time. I will reach out to you to discuss my availability and next steps.
Insurance & Self Pay options
I am an in-network provider for PacificSource, Blue Cross Blue Shield and First Choice Health. If you wish to use any of these insurance plans, you are welcome to reach out and discuss. Clients using insurance will typically be responsible for a copayment for each session (a deductible may also need to be met first depending on the specific plan).
I am an out-of-network provider for all other insurance. Please check with your insurance provider and see what benefits you may have for out-of-network care. I am happy to provide a superbill of services for you to submit directly to your insurance for reimbursement. Please be sure to review the information on the No Surprises Disclosure Act.
I also welcome self-pay clients. Depending on your needs and situation, paying out of pocket for care can have several benefits. To use insurance for therapy, I must submit a diagnosis to your insurance to demonstrate medical need. If you desire to maintain privacy, self-pay can help.
payment
I accept credit cards (including FSA/HSA) for co-pays and session fees. These are processed through my client portal and are due at time of service. Having a credit card on file is required to book an appointment.
Cancellations
Cancellations must be made 48 hours before the scheduled session or full fee for the session will be charged to the card on file.
Telehealth (Online Sessions)
I can conduct telehealth sessions through my secure client portal. I am licensed in both Oregon and Washington. If you are physically located within either of these states at the time of our session, we can work together.
